Fill-in copy of the registration form in Adobe pdf format. (requires adobe acrobat reader 3.01 plug-in for your  Windows , Macintosh, or Unix web browser). Unix users will need both the Acrobat Reader plugin and a plug-in for fill-in forms

The pdf version is a fill-in form. When loaded into acrobat reader in your browser click on the hand tool and click on each field in the form.
The bar indicates an insertion point for typing.
Type the information on the form and use the print button on your browser to print out the filled in form.
The filled in form can be saved to disk for your records.
